Sorry for the late update, but I'm trying to do like 5 things right now.

You do need to do damage roll for that ghoul, he's special.

Also, Eve, Searchlight, and Charcoal are being shot at, but they get -10 from the roll because they're in cover from the wall.

You guys should have rolled for perception to notice the guard getting shot. we are in combat. It is easy to miss such a thing. And roll for damage, my guess is that this isn't a normal enemy.

Okay, time for clarification. Combat comes in rounds. Each character makes 1 action, then I do counters/NPC actions, and then you take another round. That way I can actually post what happens without having to edit my post and update it 15 times before I can even put it up.
