You know, I did think of a way the STD could work that would be more Dramatic than comedic, but it would involve Frisk being violated and Risk taking his revenge on the offenders afterwards and that would be a little too intense IMO.

Also, Rolling a 1 means critical failure every time, even with modifiers. That's one of the universal rules of RPG dice, but to make it fare, we could have modifiers disabled for the Totem bag role.))
